Implementing effective strategies is crucial for the successful adoption of health-improving food retail interventions. To provide context on this, a novel implementation framework was used to study the Healthy Stores 2020 strategy, a novel real-world food retail intervention, focusing on factors influencing its implementation from the food retailer's perspective.
The study employed a convergent mixed-methods design, with subsequent data interpretation guided by the Consolidated Framework for Implementation Research (CFIR). In conjunction with the Arnhem Land Progress Aboriginal Corporation (ALPA), a randomised controlled trial was carried out concurrently with the study. Using photographic material and an adherence checklist, adherence data were collected for the 20 consenting Healthy Stores 2020 study stores (ten intervention/ten control) within 19 remote Northern Australian communities. Data collection on retailer implementation experiences involved interviewing the primary Store Manager at each of the ten intervention stores at the start, middle, and end of the strategic period. A deductive thematic analysis of interview data, based on the CFIR, was undertaken. From the analysis of interview data, intervention adherence scores were generated for each store visited and assisted.
Healthy Stores largely maintained their 2020 strategic plan. The 30 interviews' analysis underscored the positive impact on strategic implementation within the CFIR framework, particularly concerning the ALPA organization's implementation climate, its readiness (including a robust social purpose), and the networks and communication channels between Store Managers and other ALPA departments, which were identified as key aspects of both the internal and external domains. Implementation's triumphant or tragic trajectory frequently hinged on the capabilities of Store Managers. The intrinsic qualities of Store Managers (e.g., optimism, adaptability, and retail competence) were amplified by the co-designed intervention and strategy's attributes, balanced with its perceived cost-benefit, combined with the inner and outer environmental context, resulting in implementation leadership. In locales where the perceived advantage of the strategy fell short of the cost, Store Managers displayed less enthusiasm.
The design of implementation strategies for adopting this health-promoting food retail initiative in remote areas is informed by critical factors, including a powerful sense of social purpose, the alignment of organizational structures and processes (both internal and external) with the intervention's characteristics (such as low complexity and cost advantage), and the specific traits of the store managers. Through this study, research priorities can be redirected towards the discovery, creation, and evaluation of practical methods to integrate health-supporting food retail solutions into broader applications.

According to the latest guidelines, a TcpO2 measurement of 30 mmHg is proposed to help in confirming the diagnosis of chronic limb threatening ischemia. No clinically significant fluctuations in mean TcpO2 were observed, irrespective of whether the anterior/posterior tibial or fibular artery was patent or not.
